Job Responsibilities:

1.       Phone Assessment: Conduct initial assessments of clients' needs, including medical, social, emotional, and financial aspects.

2.       Implementing Care Planning: Develop individualized care plans in collaboration with clients, their families, healthcare professionals, and other stakeholders. This includes setting goals, identifying resources, and outlining specific interventions.

3.       Coordination: Coordinate healthcare services and resources to ensure that clients receive appropriate care and support. This may involve scheduling appointments, arranging transportation, and facilitating communication between different healthcare providers.

4.       Monitoring and Evaluation: Regularly monitor clients' progress and adjust care plans as needed. Evaluate the effectiveness of interventions and make recommendations for improvements.

5.       Documentation: Maintain accurate and up-to-date records of clients' care plans, assessments, and interactions. Ensure compliance with relevant regulations and documentation standards.

6.       Education and Support: Provide clients and their families with information, education, and emotional support to help them manage their health conditions and navigate the healthcare system effectively.

7.       Referrals: Make referrals to other healthcare providers, community resources, or support services as needed to address clients' needs comprehensively.

8.       Quality Improvement: Participate in quality improvement initiatives aimed at enhancing the delivery of care and optimizing outcomes for clients.

9.       Communication: Communicate effectively with clients, their families, healthcare professionals, and other stakeholders to facilitate collaboration and ensure continuity of care.

Qualifications:

·       Diploma

·       Minimal 3 years’ experience

·       Working hours 7am to 4pm

·       Required to work during all Malaysia Public Holiday

·       12 days Annual Leave / 11 days PH (pay in lieu or replacement leave)

·       Medical Leave as per Malaysian Labour Law

·       No medical benefits, but we have different bonus structure

·       Monthly attendance bonus, task completion bonus
